Important notice regarding Delta Dental of Missouri Individual and Family plans

Thank you for participating with Delta Dental of Missouri to provide your patients with quality dental care. We are excited to announce the launch of new Individual and FamilyTM dental plans, which are available for patients to purchase directly from Delta Dental.

Please click here to review our new plan designs that bring more options to individuals and families looking to receive quality care from you. We have five new options — Basic, Enhanced, Premium, Ultimate and Clear PlanSM. Please note the Clear Plan is not a discount plan, and you will be paid your full contracted reimbursement rate. The following changes are now in effect for members who purchased a new Individual and Family plan after September 26, 2023, (ID numbers starting with 077). These changes do not apply to members covered through an Individual and Family plan purchased prior to September 26, 2023, or any employer/group dental plan.

New phone number and interactive voice response (IVR) for inquiries regarding new individual and family members

  • For new Individual and Family plan member inquires (ID numbers starting with 077), please call (844) 764-5301.
  • For all other Individual and Family plan member inquiries, continue to reach us at (866) 991-7345.

New claim form available to download

Please note we have a new claim form for individual and family members covered by these new plans, with member ID – 077. Please use this form when submitting claims for these members.

Access to new Individual and Family plan member benefit information online

You can access new Individual and Family plan member benefit information online at my.deltadentalcoversme.com/provider. You can use the same login credentials as you use to login to the provider portal.

The process for accessing member information for an existing Individual and Family plan or any employer/group dental plan will remain the same and won’t be affected by the changes outlined above.

New brochures for your office

We will promote our new Individual and Family plans statewide, but we also want to work with you to provide these plan options to your patients who may be interested in enrolling in dental coverage. We have updated our individual dental plan brochure. You can request free printed copies for your office by emailing our professional relations team at professionalrelationsteam@deltadentalmo.com. Please include the provider’s name, office address and quantity you would like.  If you have existing brochures, please reach out for replacement copies as the contact information and the plans have changed. Please discard the old versions.

As a reminder, the changes outlined above only affect new members to Delta Dental of Missouri Individual and Family plans. These members can be identified by the first three numbers in their member ID – 077.
