Delta Dental of Missouri pre-treatment estimates for proposed dental care, formerly referred to as pre-determination estimates, are now valid for six months from the date issued.
Please submit a new claim form when services are rendered for the pre-treatment estimate and include:
- The “Estimate No.” in box 2 of the ADA claim form
- All required supporting clinical documentation and/or a copy of the Explanation of Benefits from all other carrier(s), and any reason(s) for non-payment when coordination is applied, if applicable
This change was made in response to valued feedback that we received from participating network dentists and members. The expanded timeframe is noted at the bottom of our updated “Pre-Treatment Estimate” statements.
Delta Dental of Missouri previously recommended that a request for a pre-treatment estimate be submitted to us for treatment estimated to cost $200 or more. We now suggest a pre-treatment estimate be sought for higher-cost services, such as crowns, implants, bridges, dentures and orthodontia, if such services are covered under the member’s dental plan.
